docs: set entry_maker in README's teelscope example

Update README to include an `entry_maker` configuration, enabling Telescope
to properly handle plugin-specific buffer paths ("<plugin>://<name>").
Previously, opening such paths after restarting Neovim would result in
an empty buffer.
