Changed bahvior for already distributed

• Do not create slots for already distributed elements, Fixes Starcounter/HeadsOmni#683
• Protect against trying to access parent node when disconnected on connectedCallback

Made non-displayed

so it will not get distributed in CSS Grids etc.

Fixed appending document fragment
