No data returned for some custom picklist fields

What steps will reproduce the problem?
1.Perform a Table Query or query selected rows of Accounts, Contacts or Leads
2.Include custom picklist fields
3.Run query

What is the expected output? What do you see instead?
Expect data to be extracted and be included in table. However, no data is 
returned for some of the custom picklist fields, but not others. (Three fields 
shown in attached GIF are all custom picklist fields. Two are blank, but the 
other is not.)


What version of the product are you using? On what operating system?  With
what Salesforce.com edition?
Version 16.03 - 7/14/2011; sforce Toolkit for Office V4.0
OS: Windows 7
Salesforce edition: Unlimited, Spring '14


Please provide any additional information below.
This issue manifested itself only AFTER the Spring '14 release was applied to 
our org -- did not have this issue previously.

Still experiencing this issue with Summer '14 and Winter '15 releases. Any 
resolution or suggestions?
