Better VASP uniqueness

Currently we only check uniqueness on commonName but not the legalName, which leaves open a loophole that we want to close with this issue.

  • create a signature for the legal entity that's composed of a subset of fields that must be unique together
  • add signature to the indices (note: skip this if the migration to sqlite has already happened)
  • review uniqueness and db error checking and handling
