This is an optional project to help you learn about and practice creating HTML forms. Before attempting this you should complete the FreeCodeCamp exercises on HTML Forms listed below:

• Text field
• Create form element
• Add submit button
• Radio buttons
• Checkboxes

1. You can optionally do this Mini-Project as a pair programming exercise.

2. Using CodePen https://codepen.io/pen/, try building out the below HTML form. Complete how much ever you can in one hour
