User feedback mechanisms in app

Seems like a lot of work went into developing a very nice user feedback form some years back. But I cant find anything like this on the current website.

Bringing this up because of a recent meeting with MOD/AGR curators who wanted to know where to report issues about how their data is represented/displayed in Monarch. They would like an obvious link back to the best repo in which to make a ticket for us.

The form itself is broken and there was limited bandwidth to fix it until the overhaul. Any tickets should be put directly here in this repo. From there we can reroute it if needed.
