Tutorial Approval Form is no longer available & Approval request

From this support page on package approval, there is a link to the Tutorial-only repository approval form (https://form.jotform.com/221254243344346) - which is no longer active.

At present we have an approved extension (https://github.com/climate-action-kits/pxt-fwd-edu) which also houses all of our tutorials. I'm looking to spin off the tutorials to a separate repo (https://github.com/Forward-Education/fwd-tutorials) so that content can be managed and updated separately from the makecode blocks.
