Move and re-title map on 'Submit a new report' page

• Move map to the left column of the form to sit underneath 'Categories' and above 'Refine location name'.
• Give it a heading "Report Location"
• Provide help text below the title "Click on the map to show the location for this report or type the address (including street number if known, name and city) in the box below."
• Move the 'Find location' field to above the map. (this field should be predictive searching but it doesn't appear to be working today?)
  *When people move off the location field it should autovalidate (i.e. people should not have to use the 'find location' button in order to validate an address)
• Change the mouseover for the location field to say "Please type street number (if known), street name and city"
• Remove the existing text "Search for your location using...the correct location" below the location field.
