Notify a slack channel when a visit is scheduled #30

When someone submits the Schedule a Visit form, we should push a notification to their slack channel that includes:

That it's a newly-scheduled visit
Which encampment
Which organization
Which date

The slack URL for the channel and key should be configurable per-install.
