[Settings] tools menu title mismatch between

When assigning a particular name and group in modules config, it is not properly reflected in the tool's page. The reason is because all modules use a hardcoded value in theirs menu.php files.
This issue proposes to unify all menu.php files in a common one, and take the group and tool's name from the config.
