Parent sample comment available in reports/downloads.

When using the iRecord app survey mode (and other methods with 2 sample levels), a comment can be entered at the parent "survey" level. This comment is stored in the parent sample comment field but it is not available in Elasticsearch documents so is not available for reporting and download purposes. It would be useful to add it.
